Employee OSHA Form 300 Reports

The OSHA Form 300 screen provides the ability to print the OSHA Form 300 and 300a reports using data recorded against Employee records via the Employee OSHA Injuries & Illnesses screen. Additionally, a Workers Comp Claim report can be printed from this screen.

Navigating to OSHA Form 300 Employee Report

From the menu, expand Reports, expand Employee, and click OSHA Form 300.

Incidents and Injuries Tab

The Incidents and Injuries tab will list any incidents and injuries of your employees.

There will be columns for:

  • Case No. - If there is a case number associated with the incident/injury, you can click the link to view and edit the incident/injury record.
  • Employee
  • Job Title
  • Date of Injury
  • Where the injury occurred
  • Describe the injury
  • OSHA Reportable - If the incident/injury record is not OSHA reportable, it will not be listed in the OSHA Form 300 report(s).

Filtering & Viewing OSHA Form 300 Reports

  • From the "Incidents and Injuries" tab, use the year and location drop-down menus. If a year does not have an injury/incident record, the year will not be listed.

  • Click the adobepdf.gif PDF and/or excel.gif Excel icons to download and view a report based on the year/location filter.

  • Click the adobepdf.gif PDF and/or excel.gif Excel icons towards the far-right of an injury/illness record to view additional information about the injury/illness.

OSHA Location Tab

The OSHA Location tab lists locations that will be used when tracking employee injuries/illness records.

There will be columns for:

  • Location
  • Address
  • City
  • State
  • Zip
  • Industry Description
  • SIC - Standard Industrial Classification
  • NAICS - North American Industrial Classification

From the Actions drop-down menu, you can:

  • Add
  • Delete Selected Locations

Adding an OSHA Location

  • Navigate to the OSHA Locations tab if not already there.

  • From the OSHA Locations tab, go to the Actions drop-down menu and click Add.

  • Enter the location name (required) and fill in any relevant fields.

  • Click Save.

Deleting an OSHA Location

To delete an OSHA Location, it must not be associated with an employee's injury record. If you try to delete an OSHA location, you will receive a "Cannot delete this location because there are incidents/illnesses using this location" warning.

  • Use the checkbox towards the far right of your OSHA Location(s).

  • From the Actions drop-down menu, click Delete Selected Locations.

  • Confirm the deletion of the location(s).
