Dependent Benefits Import Layout

The layout below can be used to import Dependent Benefits into the HRIS. Column Headers must be provided in the import file and the order of the columns must match the order specified in the table below. The import file must have the same number of columns as indicated in the table below, as well.

When is a Record Added vs Updated?

If an existing record is found using the key fields ([key]) identified below, the HRIS will update that record

Employee SSN Text 9 Yes [key] SSN field stored on the Employee Demographic Info screen.
Dependent SSN Text 9 Yes * Must be numeric and unique. *Must supply Dependent SSN or Dependent Birth Date.
Dependent Birth Date Date (mm/dd/ccyy)   Yes * Must be a valid date. *Must supply Dependent SSN or Dependent Birth Date.
Benefit Name Text <=50 Yes Must match Benefit Name configured within system (i.e. Medical, Dental, etc...)
Benefit Plan Name Text <=50 Yes Must match Benefit Plan Name configured within system (i.e. Preferred PPO)
Package Effective Date Date (mm/dd/ccyy)   Yes Must be valid date. Date benefits year begins (i.e. 1/1/2004)
Dependent PCP 1 Text <=50   Primary Care Physician
Dependent PCP 2 Text <=50   Primary Care Physician
