Carriers - Step 1: Providers

The form below should be used to track Provider Information specific to your account. The Primary Contact Information should generally reflect your primary electronic transfer contact (i.e. EDI Analyst, etc...)

The Provider list that is created from the addition of new carriers can then be assigned within a benefit package at the plan level of a benefit.
